Main.St., Quincy II II II II In the last two articles in "Straight Talk" I mentioned the concept of Adhesive dentistry and conservation of tooth structure when providing crowns for patients, and that among the reasons for this approach are esthetics and kindness to the skin around the teeth. In an earlier article I wrote about "latrogenic Periodontitis," or gum problems created by the dental work. In this article, I want to emphasize these concepts with two examples of patients [ have seen recently. The tirst patient came to see my daughter, Dr. Emily, with the explanation that she recently moved to this area and had been to her previous dentist requesting that she have treatment that would save her teeth. She had some expensive work done and now that she was up here, she wanted to continue with the last of this process. After examination, Dr. Emily called me in for evaluation of periodontal problems and crowns that violated the surrounding tissues. Upon further evaluation, it was noted that there were multiple periodontal problems existing and all of the crowns on her teeth were violating the principals o1" "Clean Dentistr); " or dentistry that has no negative influence on the gums or bone. This is latrogenic Periodontitis, and in this case, tooth loss will be the eventual result, which is completely contrary to the reason she went to the dentist in the first place. The second patient came to see me after reading these articles. She has been experiencing discomfort around one of her crowns and has seen her dentist multiple times only to be told that there was nothing wrong. We had a discussion regarding her concerns and I realized that this was a patient that was frustrated because she knew there was a problem and her dentist was unable to discover the cause. Upon examination, it was clear that every tooth that had a crown had inflamed tissue around it with bleeding and pus upon gentle probing and uncrowned natural teeth had no periodontal issues at all. Indeed, the tooth that she has been complaining about for some time had cement from the cementation of the crown from when it was placed to today. She has had her teeth cleaned multiple times and this chunk of cement was never removed. How could the dentist and hygienist miss it. for years even when the patient was complaining? Every crown is creating gum problems because of poor fit, poor contours, and placed too deep into the tissue. In truth, the crowns will have to be removed, the teeth contoured to correct the periodontitis, therapeutic provisional crowns will be put on, healing will be allowed for up to 6 weeks, and new final crowns will be placed that respect the surrounding tissue. The only other way to correct this is to provide aggressive advanced periodontal surgery and while this is an option, this also results in tissue loss and leaves the crowns that don't fit well and are contoured poorly, so the best option is the one outlined first. Now, remember that this is a case where the dentist could find nothing wrong. Can you believe it? Not only was this poor dentistry, but inadequate diagnostic skills were in play here as well. This is a very common occurrence, unfortunately. If you or someone you know are frustrated with chronic discomfort or crowns with bleeding gums, bad taste, red gums around crowns that are esthetic problems, or have been told that you need to "brush more" or are "allergic to the metal or other material in the crown," realize that there are ways to treat this absolutely. Also, realize that the reason for these problems are almost NEVER about the materials in the crowns but almost ALWAYS about how the crown is done, contoured, shaped, and placed. This is Iatrogenic Periodontitis pure and simple. Treatment is available but the dentist must be trained and experienced to be able to provide it predictably!
