Thank you for your continued support. ****STRAIGHT TALK FOR THE CONSUMER**** 1 have decided to make this a weekly series for a while as I develop a background for future articles! To begin this series, I would like to discuss some of the fundamentals of dentistry, not only what relates to what we do forpeople, but philosophical fundamentals as well. As I write this I will bring up some procedures as .examples so be patient for all of these will be fully explained in future articles dedicated to specific" procedures. Consider the idea that conservative dentist~ does not mean doing the least invasive or least expensive treatment; it means providing treatment that is the most conservative for the long run. For example, if the dentist sees a tooth with a filling that compromises the tooth's structural in- tegrity, he/she might feel that to crown the tooth when there is no symptom is too aggressive and decide that the most conservative treaiment is to wait until symptoms arise and then treat the tooth. In reality however, the fractures that are happening can seriously compromise the tooth, so that by waiting, a catastrophic event can occur creating the need for a root canal, or periodontal crown lengthening surgery, or even the ability to save it. So the most conservative treatment would not be to wait, but to treat the tooth before it becomes, symptomatic. The point is that some- times the most aggress!ve treatment initially will be much more conservative in the long run. It is important that the patient has this option given and, to not give this option is to do the patient a disservice by not allowing them to make their decision based on a complete and proper discussion. As you can see, there is much more to this story than you may be aware. For nearly 30 years I have practiced in Plumas County, wonderful years bringing the best that I can to my patients. This has not been easy for it has taken years of education with excellence at the forefront of intent. My focus on the treatment of gum disease (Periodontal Disease), for example, made me somewhat controversial from time to time for I was considered too aggressive in treatment. But the years have born out the importance ofthis approach, and indeed, many dentists are actually not aggressive enough when it comes to this disease. Those that have criticizedmy approach have been shown by research tO have been actually very wrong. Now that we know that chronic inflammation that accompanies Adult Periodontitis can lead to twice the risk of coronary heart disease and stroke (and many other systemic diseases), there is absolutely no argument available against aggressive thorough treatment of this condition whenever encountered in our patients. Indeed, this will be a subject that I will cover very thoroughly in coming articles that will include the phenomenon of Supervised Neglect; what this means, why and how it occurs, and how to avoid it. Interestingly, there is a type of Periodontitis that is caused be file way some dental treatment js done and this also has a name:. Iatrogenic Periodontitis. Now, that is interesting, isn't it? These are subjects that you will never hear discussed openly for it is one of the hidden secrets of the dental profession, a dark secret, but one that has tremendous implications in your overall systemic health and it is completely supported by research. So, I will continue to bring the story of dentist~ to all of you, it will take some time to set the stage for discussion of specific issues you may encounter, and I will do my best to bring it to you in a way that makes sense. By setting the stage, articles that I write on specific conditions and treatments will then have more meaning for you. This is about bringing dental education to those who would like to know what is really going on and how to understand what you are being told regarding your oral health when you vis!t the dentist. Trust is paramount and everyone wants to trust the dentist, but even the subject of trust is a deep one; to be trustworthy one must be worthy of the trust. Now, that is a very big responsibility for us! Integrity requires us to stay on top of the research and fully un- derstand the updated methodology and science behind what we tell people because believing in somethingin and ofitseifd0es not make it right: We have to know the subject well enough to be sure we are trustworthy when we counsel our patients. Otherffise, refer it to someone who does know. That is our responsibility to you. There simply is no way around that one...
